We’re thrilled to partner with your surgery to provide top-quality workwear and uniforms. Follow this quick guide to get started, and soon you'll be outfitted in fresh, new uniforms.

Surgery Onboarding Process

1. Access the Portal
   Visit PBG Members Portal and search for “uniform” to receive the demo login for the Futurform PBG portal.

2. Login and Explore
   Use the demo login to view the Futurform Portal. Please note that pricing won’t be visible until you log in.
   If you'd like to order samples, you’ll need to create an account by completing the form here. Sample garments are chargeable but refundable upon return.

3. Account Creation
   Once your form is submitted, we’ll set up your surgery’s account and provide personalized login details for the portal.

4. Placing an Order
   When you’re ready to place an order, you can do so via the portal at Futurform Portal, by phone at 01373 801262, by email at pbg@futurform.co.uk, or via live chat on our website.

Order Process

1. Plain Garment Orders
   We recommend placing plain garment orders through the portal for the quickest service. Alternatively, orders can be placed via phone or email. Plain stock will be shipped out within 1-2 working days.

2. Decorated Garment Orders
   For decorated garments, we encourage surgeries to reach out by phone or live chat first. We will need a copy of your logo for digitisation, along with a photo of any previous embroidered garments so we can match the decoration.

3. Repeat Orders for Decorated Garments
   Once your logo is digitised, future decorated garment orders can be placed directly via the portal, where you can select your saved logo.

4. Order Review
   Our team will manually review your order for stock availability and garment suitability. If we have any questions, we’ll contact your surgery before processing the order.

5. Delivery Times
   Embroidered uniforms typically arrive within 10-14 working days if the items are in stock. If stock is unavailable, delivery may take up to 21 days, and we will notify you as soon as possible.
